Intermountain expands heart failure program partnership with Story Health unknown
Salt Lake City based-Intermountain Health has expanded its heart failure program in partnership with digital health company Story Health.
The expansion follows the partnership's successful pilot program that incorporated virtual and asynchronous care into heart failure treatment, according to an Aug. 21 news release from Intermountain.
A hypertension pilot program was also launched as part of the partnership earlier in 2024. The program focuses on controlling blood pressure levels.
"Intermountain has long been a leader in developing innovative models of care for treating complex patient populations. However, we need to continue to close remaining gaps in care to truly deliver best-in-class cardiology care," Kaley Graham, RN, executive director of Intermountain Health’s heart and vascular program, said in the release.
